New Member Forms

New Member Eligibility Form

FSL requires all potential new members to fill out a eligibility form. FSL verifies you are above a 2.50 GPA then informs chapters if you are eligible. chapters are not be able to see your GPA

New Member Exit Interview Form

This form is filled out by the new member who wish to drop their organization. This form should be filled out after the new member has declared their intention to drop with their chapter's new member educator

Membership Acceptance Form

Congratulations on getting a bid/invitation from an a Chapter fill out this form to accept your invite.

New Member Educator Forms

New Member Education Calendar

The new member education form must be filled out before the FSL cutoff date in accordance with FSL guidelines. This form declares all events that pertain to your new members educaiton proccess

FSL Initiation Form

The initiation form must be completed after new members have be successfully initiated into your organization. The Cutoff date is listed in the FSL guidelines for each semester.
